INSTRUCTIONS TO FILL THE FORM:

Please go through the following instructions before filling the form:

• Form should be filled in CAPITAL letters only
• Please write – N A – where it is not applicable
• Kindly note that you need to take print and submit the duly filled in Admission form at the School Reception office (including doctor's seal and signature on health form) during final stage of admission. Please write the full name of the student and the class for which admission is sought, behind the photographs being submitted along with the hard copy of this admission form during final submission.
• Religion/Caste information is required for submission of records to the Government and will be kept confidential.

1. The information furnished by me/us in this Admission Form is true to the best of my/our knowledge.
2. I/We have filled this Admission Form after reading and understanding completely the rules mentioned in the Admission Procedure form.
3. I/We declare that my/our son/daughter has not been debarred from studying in any school or from appearing in any examination in the previous school.
4. I/We declare that all previous medical and psychological histories are correctly reported on the Admission Form.
5. I/We understand that the school transport will be provided on specified routes/stops only.
6. I/We declare that the school holds the right to use the student’s photo/video for any publicity material. Parents cannot use the school’s image for posting on any social media unless with school’s written permission.
7. I/We acknowledge that the submission of Admission form alone does not guarantee admission. The school reserves the right to keep the admission on hold until all documents mentioned in the checklist are submitted.
8. I/We agree to pay the applicable fee on time and to abide by the payment options outlined in the fee schedule. I/We fully understand that the amount paid towards admission is non-refundable.
9. I/We acknowledge that the school will take reasonable care and exercise due diligence within its premises, during school hours, during school activities and will bear no responsibility should the student exercise any reckless and/or careless behaviour that may endanger his/her safety and that of others around causing harm/injury to himself/herself and others.
10. I/We understand that in serious instances of breach, e.g. bodily harm to other students/teachers, damage to school property, the student may be asked to leave the school.
11. I/We acknowledge that the school will not be responsible for loss/damage of any personal belongings brought to school by the student.
12. All students are required to return any material/book etc borrowed from the school, before they leave the school.
13. I/We will not use social media to send abusive messages to/about parents or teachers, post defamatory status about parent/student/teacher/school or use social media to complain or post any grievances about the school’s values and methods.

I grant permission for the school nurse to administer non-prescription medicines like paracetamol (anti-pyretic), anti-allergic and antispasmodic. I also grant permission to administer first aid till such time the parent/guardian can pick up student from the school. Vishwa Vidyapeeth School will not be held responsible for any complications arising out of lack of information or awareness of pre-existing illness/allergies.
